• Your referral slip and x-rays from your referring dentist (if your dentist takes digital radiographs, please have these transferred to our office prior to your appointment) If you do not have x-rays, please let us know when you make your appointment.
• If you have dental benefits, please bring any forms or documentation with you to the appointment.
• When you arrive you will need to complete Patient Registration and Medical History forms. Please bring a complete list of any medication and supplements that you are taking.

All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
